APPENDIX A: “Getting to Know You” Survey

Hello, students! Building community in our class and making sure each student feels a welcome part of that community is a priority for me. Your input on this survey will help me get to know you and assure I am able to meet your needs as well as those of the rest of the students in the class. Please take a few moments to complete this quiz/survey. This should take no longer than 10 minutes.

Your responses will come directly to me and will be kept confidential. However, feel free not to answer any of the questions you are uncomfortable responding to (do not worry about the points, you will still get full credit 🙂).

If there is anything you want to know about me, please ask. I have also shared many details about my background and interests in the Getting Started module in Canvas.
